La Léproserie de Gisors fut fondée en 1210 par Jean de Gisors, la chapelle fut remaniée tout au long du Moyen Âge. D'une superficie de 110 m², elle se compose actuellement de deux parties rectangulaires : l'une, construite en moellons de pierre de taille calcaire, l'autre, un peu plus étroite, à colombages avec pans de bois.
En 1967, l'hôpital céde l'ancienne chapelle à la ville pour le franc symbolique. Classée au titre des Monuments Historiques en 1992, sa restauration commence en 1996. De 1998 à 2010, Dado (Miodrag Djuric), célèbre artiste peintre monténégrin, en peint l'intérieur du "Jugement Dernier".
